Sounds like its Worn Out...You can send it to me for Disposal ....LOL
Worn parts, loose Magazines, weak springs, chipped ejector ...It could be a number of things and impossible to diagnose on the Internet...

Sounds like a broken extractor lip to me. Try replacing the extractor. Check the ejector while you have the bolt apart. That could also be the source of the problem.

Means the empty case is sticking straight up in the action/chamber area. Either open or primer end up...

A broken extractor lip or worn ejector might explain the lack of grabbing another/next round, but I don't think it explains the stovepiping. I'd have to agree with mooseman as there's many reason for the cause of both from bad ammo to worn parts and as he stated, it's hard to diagnose on the internet.

Take it or ship it to a qualified reputable gun smith. MHO anyhow...

Actually it does explain some stovepiping. The rim slips out from under the extractor and the ejector flips the case straight up instead of to the side. Stovepiping is common with broken extractor lips and is one of the most common symptoms. And for whatever it is worth, I am a carbine smith........
