Too much powder !

I'm not sure I remember who, but I do remember he collected a gash in his forehead and stunned himself. He's lucky it wasn't worse. Was it ever determined what happened? I'd put my money on an accidental double charge in a handload, but I wouldn't rule out a squib load that blocked the barrel and generated enough pressure to blow the top strap off when the next round was fired.

I don't even reload. Wasn't my gun...but Forty has one exactly like that gun and he reloads

I'm kind of glad I've never personally seen a pistol like that. I was unfortunate enough to be there when a guy's rifle blew up and took a couple fingers with it.

Never did know why it went, though.

So far, I have seen two rifles Kaboom - an AR-15 and a Ruger Mini-14. The AR shed no blood, just blew down through the magwell.
The Mini-14 lost a lot of wood forward, and splinters gashed the owner's hand.

Consensus was, the AR-15 fired out of battery, and the Mini-14 owner used questionable handloads.

I've never seen a handgun Kaboom - yet......

My vote, Forty XDM is what happened. You kind of gave it a way with the title. And, I really don't think you'd publicly blame anyone here.

How did two rounds go off at the same time? Both appear to have definitely exploded from the inside out. What gives? lol

I think it could have been not enough powder, light load of powder in a large case, all the powder lays on the bottom of the case and it ignites all at the same time,instead of burning from the primer to the bullet, you get a high pressure spike. Its called detonation!

and it wasn't too much powder as it was factory loads. problem was with the gun (cylander #2), not the shell.
