I remember filling out a form like this one, shortly after Bill Clinton became President. The one's that filled it out and sent it in received a brand new $2.00 once a month with his picture on it !!!! |

Stitchclimber, Where did you get that form? I believe it is illegal. I am pretty sure that Arkansas has the same type law as Oklahoma. No State form requesting information can be over four lines long. This law was passed to protect our citizens from eye strain and prolonged headaches from forced reading. Lol
