Question Having Some Trouble

#1 At 25 yds my carbine puts every round exactly at point of aim. At 50 yds its about 5 inches high and at 100 yds about 10 inches high. The rear sight is set at the 100 yd mark. Is it my sight picture or could it be something else?
#2 Ive shot 400 rds through my carbine and 2 times the bolt has slid out of the charging handle. I can put it back in with out taking the gun apart in about 20 sec. but its kinda annoying!!!!!!!!! Any Ideas. Thanks

What kinda gun and ammo are you shooting? Could be ammo too hot. If gun is new I doubt it would be from being worn. You ar USMC so I know you know what a good sight picture is.??????????????????

Question

Check the front sight, maybe somebody Bubba'd it?
Is the rear sight the later adjustable one, or the early flip-up style?
Is this a genuine GI carbine, or a later commercial clone? I had the bolt on my IAI clone carbine jump like that once, but otherwise it's been a super little gun.
You might have a good 'smith check it for excessive wear, especially if it's an older gun.

My carbine is a clone (EFM). It has the adjustable rear sight and the gun is well used. I've been shooting UMC ammo and yes I do know what a good sight picture is but hey, you never know!!!!!!!!

If you have a USGI slide on it, you could simply replace the slide. But I would check barrel/receiver alignment first. If the two are not properly aligned, the slide handle can slip off the bolt lug in just the manor you describe. If there are witness marks on the bottom of the receiver and barrel, make certain that they are aligned with one another.
