Does anyone here have a copy of Form 5300.38; Application to Ammend Federal Firearms License? The form is unavailable from the ATF's website and has been unresponsive to my emails.

There should also be a copy in the back of your "Green Book".

OK... it's been a reasonable amount of time since I sent in a change of physical address to ATF. Question...will they send me an amended license shortly or will they just wait and I'll get the updated one on the next C&R renewal? Anyone know?
