I'm not sure I remember who, but I do remember he collected a gash in his forehead and stunned himself. He's lucky it wasn't worse. Was it ever determined what happened? I'd put my money on an accidental double charge in a handload, but I wouldn't rule out a squib load that blocked the barrel and generated enough pressure to blow the top strap off when the next round was fired.
I'm kind of glad I've never personally seen a pistol like that. I was unfortunate enough to be there when a guy's rifle blew up and took a couple fingers with it.
Never did know why it went, though.

So far, I have seen two rifles Kaboom - an AR-15 and a Ruger Mini-14. The AR shed no blood, just blew down through the magwell.
The Mini-14 lost a lot of wood forward, and splinters gashed the owner's hand.
Consensus was, the AR-15 fired out of battery, and the Mini-14 owner used questionable handloads.
I think it could have been not enough powder, light load of powder in a large case, all the powder lays on the bottom of the case and it ignites all at the same time,instead of burning from the primer to the bullet, you get a high pressure spike. Its called detonation!
