Remember it goes to your most local CLEO, so if you live in town it goes to your police chief, not the county sherrif.
I've always sent mine to the Chief's name, care of the police department's address.
I send the copy that's intended for the CLEO, and a letter informing him that you're applying for a collector's license, and that you're not a dealer and will not be using this license to perform business, but only to better your personal collection, which is kept in a secure, locked place in your home..
